Endodontic Retreatment
In Laguna Niguel, CA

What is endodontic retreatment?

Endodontic retreatment is a specialized procedure performed when a tooth that has previously undergone root canal therapy does not heal as expected or develops new problems over time. While root canal treatment is highly successful, issues such as untreated canals, new decay, a compromised restoration, or complex anatomy can sometimes lead to recurring infection or inflammation.

Common reasons for retreatment include:

  • Persistent pain or swelling after a root canal
  • Untreated or missed canals
  • New decay or damage that exposes the tooth to bacteria
  • A leaking or broken restoration
  • Complex anatomy or canal obstructions that prevent proper healing

How retreatment works:

During retreatment, Dr. Gharavi will carefully remove the existing filling materials, thoroughly clean and disinfect the canals, and then refill and reseal the tooth with biocompatible materials. Advanced technology, such as in-office CBCT 3D imaging and surgical microscopes, will be used to enhance precision and visualization throughout the process.

Many insurance plans provide coverage for endodontic care, making retreatment an affordable choice for maintaining long-term oral health.

Save Your Tooth

Endodontic retreatment helps save your natural tooth so you can maintain a healthy smile and full bite function.

The Endodontic Retreatment Process

Step 1: Consultation and Disassembly

Your first step is a consultation with Dr. Gharavi to thoroughly examine your tooth and discuss your treatment options. This includes reviewing your dental history and using advanced 3-dimensional imaging with our in-office CBCT machine to accurately evaluate the tooth and surrounding structures. If endodontic retreatment is recommended, Dr. Gharavi will review an individualized care plan with you in detail so you can make an informed decision about your treatment. Once you choose to proceed, Dr. Gharavi will carefully reopen your tooth and disassemble any existing crown, post, or filling material to gain access to the root canals for retreatment.

Step 2: Cleaning and Disinfection

Once you are fully comfortable, Dr. Gharavi will use an advanced dental operating microscope to carefully address the source of non-healing. This involves removing any old filling material, locating areas of concern, and thoroughly cleaning the root canal system with specialized instruments and disinfectants. Using advanced laser techniques, the canals are disinfected to address any areas that may have been missed during the initial treatment.

Step 3: Filling and Sealing

Once your tooth has been thoroughly cleaned and disinfected, the root canals will be filled with a safe, biocompatible material that helps support the tooth and promotes healing. After the canals are filled, a temporary restoration will be placed to protect the tooth until your follow-up visit with your general dentist, who will place a permanent restoration. This follow-up is important to ensure the long-term strength and function of your tooth.

If you have previously undergone root canal treatment and are experiencing persistent tooth pain, swelling, or other signs of infection, you may be a good candidate for endodontic retreatment. Dr. Gharavi will evaluate your specific case during a consultation to determine the most appropriate course of action and develop an individualized treatment plan.

Sometimes, a tooth may not heal as expected after initial treatment due to factors such as:

  • Untreated narrow or curved canals
  • Complicated canal anatomy that was not detected previously
  • Delays in placing a crown or restoration after the original root canal
  • A restoration that did not fully prevent contamination

New problems can also compromise a previously treated tooth, including:

  • New decay exposing the tooth to bacteria
  • A loose, cracked, or broken crown or filling
  • Tooth fractures

Whenever possible, Dr. Gharavi will prioritize saving your natural tooth, with extraction reserved only as a last resort. Teeth that undergo successful retreatment can continue to function normally and thrive for many years.

Dr. Gharavi uses the latest technology and offers a variety of sedation options to make retreatment as comfortable as possible. Most patients report minimal discomfort during the procedure, and retreatment often results in less post-procedure pain compared to having a tooth fully extracted. The goal is to relieve persistent tooth pain while preserving your natural tooth, making the recovery process smoother and more manageable.

Many dental insurances do cover at least a portion of your retreatment costs. However, we recommend getting in touch with your provider to get a better understanding of your benefits and coverage.
